At Steele Memorial Medical Center, our mission is to provide compassionate, high-quality healthcare to every person we serve. As part of that commitment, we conduct a Community Health Needs Assessment (CHNA) every three years to better understand the health challenges, barriers, and priorities of our region.

What is a CHNA?

A Community Health Needs Assessment is a systematic process that evaluates the overall health status and needs of our local population. It incorporates community surveys, public health data, and feedback from residents, healthcare providers, local leaders, and organizations. This allows us to identify key health concerns and opportunities for improvement.

Why it Matters

The CHNA informs how we plan our services, direct our resources, and collaborate with partners. It helps us:

  • Prioritize health concerns using both data and community input

  • Shape future programs and outreach initiatives

  • Support grant funding requests

  • Improve access to care and health equity

  • Build long-term partnerships for community health improvement
