What to Bring
During your appointment with Steele Memorial Clinic Financial Services team, we will help find the best programs for you. Our team members will ask several questions to discover what you and your family are eligible for and an application process will be initiated. Please be assured that all responses will be kept completely confidential.
Discounted/sliding fees are based upon federal poverty guidelines, and patient eligibility is determined by annual income and family size. Schedules are established and implemented to ensure that a non-discriminatory, uniform, and reasonable charge is consistently and evenly applied. The site does not discriminate in the provision of services to an individual:
- because the individual is unable to pay;
- because payment for those services would be made under Medicare, Medicaid, or the Children’s Health Insurance Program (CHIP); or
- based upon the individual’s race, color, sex, national origin, disability, religion, age, sexual orientation, or gender identity.
Patients are required to provide documentation in order to determine eligibility for Steele Memorial's Sliding Fee program. Please provide as many of the following documents (one per category) as you have available:
- W-2’s and/or Social Security statement (Form 1099 received at beginning of year)
- Last year’s tax return (if applicable)
- Pay stubs – Last two current
- Copies of IDs
